 5. Physical Examination Report (you may download it from the website of Chinese embassy in your country, scanned in PDF format, valid within six months).

 6. A Letter of Recommendation from your current boss or colleague (scanned in PDF format)

 7. First page of Passport (scanned in PDF format)

 8. A digital passport-size photo (scanned in JPG format, submit four photos when arrival.)

 9. Fill in the Application Form for Study at Shandong Agricultural University (scanned in PDF format)（attached in this email）

 10. The authentication letter of your finial degree issued by the Embassy or Consulate of China (scanned in PDF format)

 11. The authentication letter of your no criminal proof issued by the Embassy or Consulate of China. (scanned in PDF format)
